Mission
Connecticut Community Care, Inc. (CCCI) identifies choices and provides services to help people of all ages, abilities and incomes to live at home.
Who We Are
CCCI is the premiere and largest care management service for the elderly and individuals with disabilities in Connecticut. We are dedicated to assisting clients to remain independent and living at home for as long as possible.
For more than 25 years, CCCI has helped thousands of individuals stay at home through care management.

The Connecticut Home Care Programs (CHCP)
CCCI serves as an Access Agency to the public home care program in 124 towns throughout Northwest, North Central and Eastern Connecticut.
| My mother was a client of CCCI’s for over 10 years. Her health had declined but I finished my walk out basement to make it accessible for her to live in my home. Mom was in and out of various nursing homes over the past few years due to different surgeries. We know, for her, there truly was “no place like home.” We had the same care manager for this entire time and she was wonderful.
