We evaluate the client’s living situation, medical, cognitive, and emotional status, existing support systems, environment, and financial situation. In addition to geriatric assessments, we conduct evaluations for individuals with disabilities who need support services in order to live safely at home. Our Care Managers review options, develop a plan of care, and provide written reports of their findings. Assessments include:
Telephone/Office ConsultationsThe search for resources to fit the varied needs of an elder or loved one with disabilities can be a stressful and time-consuming process. The CMA team is available by telephone or for an office consultation to discuss eldercare resources and the range of available services for those who need support to live safely and independently in their own homes or in an alternative living situation. They can also discuss strategies for families and caregivers dealing with unique challenges, provide caregiver resources, and help mediate family differences. Coordination & MonitoringOnce a plan of care has been developed, we can assist with locating, coordinating, and monitoring services to meet client needs. Care Managers are available 24 hours a day, seven days a week, to provide case management and ensure that the care plan runs smoothly and meets the changing circumstances of the client and family. Long-Distance CaregivingFamilies dealing with long-distance caregiving and dementia care issues have special needs. We conduct a comprehensive geriatric assessment, find and coordinate the support services needed, and act as the family's eyes and ears locally. Our professional care managers oversee all of your loved one's needs when you can't be there. If you’re caring for someone with dementia, we can help you understand the illness, find appropriate caregivers, and prepare for the future Resource InvestigationWith extensive knowledge of the many resources available, whether it’s medical equipment, home care support, physicians who make house calls, or recreational activities for elders and adults with disabilities, we identify the options and support services for our clients that provide the highest quality of care at the most affordable rates. Assistance with Entitlements and Conserving AssetsNot sure where to turn? Confused about Medicaid and Medicare? CMA helps clients and their families locate state and federal entitlement programs and other appropriate funding sources. We assist with paperwork requirements and help clients find resources to conserve their assets. Advocacy and Probate Court Expert TestimonyWe advocate for the best interests of our clients and can serve, if necessary, as conservator of person. Our Care Managers are often asked to conduct objective geriatric assessments, prepare reports, and provide their expert opinion on matters related to probate court.
