"We are very pleased to have found a care manager from Care Management Associates.  She's someone who will be a local point of contact for the family, who will coordinate communication between the aides, the assisted living caregivers, and the doctors.  She will make sure that someone goes to Mom's doctor appointments with her and will be there at the hospital if Mom has another emergency, keeping track of her medical history and doctor's instructions, and speaking up for her within the meat grinder of our medical care system.  Having her there with Mom means that we can get complete information about doctor visits and Mom's status without having to be there for every appointment, and now Mom has a spokesperson who will arrive at the hospital, night or day, if she suddenly needs medical attention.  We are all VERY glad to know that we now have a professional available to help Mom with communication during both routine and emergency medical situations."
