Care Management Associates (CMA)

Care Management Associates, the private division of Connecticut Community Care, Inc., helps elders and adults with disabilities find the resources needed to live as independently as possible.  We provide comprehensive geriatric assessments and case management. Our care managers are nurses and social workers with years of experience in care management. In 2007 and 2009, we were chosen by Connecticut's attorneys and law firms as "The Best" in Home Care through a survey done by the Connecticut Law Tribune. 

Whether you are a family member concerned about your aging parents, an adult with disabilities who needs a helping hand in order to live at home, or an attorney, probate judge, physician, bank trust officer, or financial planner working with an elderly client, we can help.  Our professional staff assesses the individual’s unique needs, develops a plan of care, coordinates services, and ensures peace of mind for the whole family.
