What is Care Management Associates?
Care Management Associates (CMA) provides professional care management and personal assistance services to individuals on a private pay basis. CMA is a division of Connecticut Community Care, Inc. (CCCI).
What is Connecticut Community Care, Inc. (CCCI)?
CCCI is the largest access agency for the publicly funded Connecticut Home Care Program for low-income elders and adults with disabilities. CCCI is the parent company of CMA.
Are there any age or income restrictions on CMA’s private pay services?
No, we can provide services to any individual.
Are CMA’s services available statewide? Out of state?
Our private services are available anywhere in Connecticut. We also offer long distance care management services to individuals residing in other states, and to Connecticut-based clients with out-of-state caregivers.
Are CMA services covered under long-term care insurance policies?
Yes, both CMA’s care management services and homemaker-companion services are covered under many long term care insurance policies.
Does CMA offer direct care services?
As a registered homemaker-companion agency (#HCA.0000162), CMA offers both certified care managers and highly qualified personal assistants.
What is the role of the care manager?
The care manager is the health care ‘quarterback’ in each case, responsible for:
- Assessing the client’s situation
- Developing an appropriate and cost-effective plan of care
- Coordinating with direct service providers such as home health agencies, companion and homemaker services, adult day care centers, and others
- Monitoring and modifying the care plan as the client’s needs change

How can CMA’s care managers help me in my professional practice?
Professionals with legal or fiduciary responsibility for elders or individuals with disabilities typically do not have health care experts on staff. When health care issues arise with clients, CMA care managers can be a trusted resource. Clients can be referred directly to us, or we can contract with you as an extension of your services.
What role do CMA care managers play in probate court matters?
Probate courts may request an assessment, or may appoint a care manager as conservator of person. Care managers may provide expert testimony in court hearings.
How can CMA care managers help me when I go to probate court?
Our care managers are most helpful in situations where there is conflict. For example, siblings may disagree about a parent’s capabilities. Attorneys who have worked with our care managers comment that, “(your) assessments have been helpful for all involved,” and that “probate judges give a lot of weight to (your) assessments.” In situations where an objective opinion is needed or others involved in the case are not health care experts, a care manager can provide the needed expertise.
What qualifications do CMA care managers have?
Care managers are either nurses or social workers with extensive experience in case management. Our CMA care managers are certified by the National Association of Certified Care Managers (NACCM).
What information is included in a typical care manager assessment?
The care manager’s comprehensive assessment evaluates multiple areas of the client’s situation. The assessment evaluates the following:
- Reason for referral
- Medications
- Health conditions
- Activities of Daily Living (ADLs) such as walking, dressing, eating and taking medications
- Instrumental Activities of Daily Living (IADLs) such as driving, meal preparation and housework
- Memory
- Mental status
- Support system
- Psycho-social capabilities
- Environment
The assessment report includes a comprehensive summary of the evaluation as well as a care plan with recommendations and options for care.
How much does a typical assessment cost?
Depending on the complexity, the cost of the assessment ranges from $500-$800. Phone and office consultations are also available.
What services do CMA personal assistants provide?
Our highly qualified personal assistants provide a wide range of services including: appointment scheduling and transportation; companionship/social and emotional support; meal preparation/assistance with eating; personal care duties such as bathing, dressing, grooming, transferring and range of motion exercises; light housekeeping; medication monitoring; shopping; pet care assistance; mail management; leisure activities, and more.
What if additional services are needed?
If additional health care services are needed, CMA has access to the extensive resources of our well-established network of more than 200 service providers. The advantage of working with CMA to coordinate these services is that our care managers can provide ongoing oversight of each case and modify the care plan as needed.
What happens if there’s a problem in the evening or on weekends?
Unlike many agencies, our personal assistants as well as clients and their families have 24/7 access to our care managers when questions or concerns arise.
How do your rates compare with other similar agencies?
Other agencies have variable pricing depending on the services provided. We offer flat rate pricing for personal assistants. Because we pay our personal assistants more than most other home health agencies, we are able to attract and retain the best.
