For ProvidersConnecticut Community Care, Inc. (CCCI) contracts with the Department of Social Services (DSS) in administering the Connecticut Home Care Program (CHCP) in 124 Connecticut towns. Through this program, CCCI contracts with more than 200 direct service providers to supply a broad range of services, including: visiting nurses; adult day care; meals and transportation; homemaker and companion; therapies; emergency response; and more. To help ensure that the highest quality of service is provided, CCCI performs ongoing clinical and financial audits of our contracted providers. These audits not only ensure contract compliance but also serve as an educational process by helping providers prepare for audits conducted by DSS. CCCI monitors client satisfaction through periodic surveys and feedback from our professional care management staff, taking the necessary steps to correct identified problems. CCCI also offers ongoing support to our provider community through informational forums, publications and one-on-one training. CCCI providers are responsible for providing quality care to more than 12,000 elders and individuals with disabilities served annually. The partnership between CCCI and its providers enables clients to remain at home for as long as possible.
