Most people prefer the option of living in their own homes. At Care Management Associates (CMA), the private-pay division of Connecticut Community Care, Inc. (CCCI), our care managers and staff help our clients – the elderly and people with disabilities – live as independently as possible in the homes of their choice.
What do CMA care managers do?
CMA’s certified care managers view clients from a holistic perspective, assessing their situation in all areas: family and community supports, medical, emotional, psychosocial, cognitive, financial, and environmental. Based on this comprehensive assessment, care managers develop a plan to address the client's needs; recommend and coordinate services and providers; and modify the home care plan as the client’s needs change.
Our certified care managers are leaders in the field with more than 60 years of combined experience in health care.
Nan O’Brien, NACCM Certified Care Manager, BA, CMC
Nan joined CCCI as a care manager in 2000. She received her BA degree in Sociology from the University of Hartford. Her experience working with elders and adults with disabilities spans 30 years. Nan works with clients served through CMA’s contract with the National Multiple Sclerosis Society, Connecticut Chapter. Based on the success of this program - clients who have successfully returned to active roles as spouse, parent, employee, friend, parishioner and citizen - a state-funded home care pilot initiative for persons with neurological disorders is moving forward. Nan presented a program on Care Management Assistance for Persons with Disabilities at the 2009 AARP National Conference on Diversity. Prior to her position as a care manager, Nan worked in operations management for a home care services provider agency in Connecticut and Massachusetts.
Rayda Bouma, NACCM Certified Care Manager, BS, RN, CMC
Rayda is a certified nurse care manager with more than 20 years of experience. She received her nursing diploma from Blodgett Memorial Hospital in Grand Rapids, Michigan and her BS degree from Central Connecticut State University in New Britain, CT. She received her care management certification (CMC) through the National Academy of Certified Care Managers (NACCM). She has taught extensively; represented CCCI at local and national conferences; and has provided expert witness testimony regarding long-term care issues. She coauthored an article “Attorney-Case Manager Collaboration in the Conservatorship of Older Adults,” published in the Journal of Case Management.
Prior to her current role as care manager, Rayda worked as a neurosurgical clinical specialist and assistant head nurse at Hartford Hospital where she was actively involved in designing and developing the Spinal Cord Injury Committee. She has been a member of the board of directors and president of ABLE, an organization designed to provide support for developmentally disabled adults living in the community.
Donna Gaudio, NACCM Certified Care Manager, BS, RN, CMC
Donna has been a care manager for more than 10 years. She received her BS degree in secondary education from Central Connecticut State University and her AD/Registered Nurse from Tunxis Community College. She has completed graduate courses at Quinnipiac University including Immunology, Endocrinology, and Epidemiology. Donna is certified as a geriatric care manager through the National Association of Certified Care Managers, and was previously certified as an infection control practitioner. Donna’s prior work experience includes: critical care nurse at Yale New Haven Hospital; inpatient psychiatric/research nurse at the Connecticut Mental Health Center; community nurse with the New Haven Visiting Nurse Association; rehabilitation nurse at Gaylord Hospital; developmental disabilities nurse/supervisor and clinical instructor with the State of Connecticut; and director of infection control and quality assurance at the Alzheimer’s Resource Center in Plantsville, CT.
Charlene Breen, Clinical Associate, BS
Charlene has 20 years of experience working with elders and adults with disabilities and their families. She holds a BS degree in education from Eastern Illinois University. As clinical associate, Charlene takes initial inquiries and provides information to prospective clients. She assists care managers with resources, coordination of services and general client support. Charlene also provides information and resources on care management to families, health care professionals, the legal community and others. Charlene has worked in administration, therapeutic recreation and admissions in extended care facilities. She has also acted as a community liaison and marketing director for assisted living facilities.
