24/7 Emergency Triage Service

CCCI is contracted with the State of Connecticut, Money Follows the Person (MFP) Program to establish and maintain a 24/7 Emergency Triage Service. The service is available 24 hours a day / 7 days a week and is staffed by a Triage Specialist. The service is available to all MFP consumers during their first year in the community. Consumers have access to the service through a toll-free telephone line.

Goals of the 24/7 Emergency Triage Service

  • Provide an additional level of security for participants transitioning under MFP
  • Reduce reliance on acute care facilities
  • Triage calls and guide participants to resolution
  • Document work force reliability issues

When MFP consumers contact the Triage Service, they are greeted by a Triage Specialist. The Triage Specialist explores the present problem to assess the level of emergency.

Is the situation:

            • urgent or does a 911 call need to be placed?
            • critical, but can wait (how long and with what result)?
            • not an emergency?

When there is an unusual or unexpected breakdown in a critical service, the Triage Specialist provides the guidance and tools to coach the caller to a resolution.

Once the level of emergency is determined, the Triage Specialist will guide participants to a resolution, including:

  • exploring their back-up plan.
  • exploring alternative options until the situation is resolved.
  • education regarding emergency planning, back-up supports and options.
  • intervening directly if participants are unable to do so themselves.
