Mission
Connecticut Community Care, Inc. provides care management services to
older individuals and other persons with similar chronic care needs and
their caregivers, to assist them in remaining as independent as possible
in the most cost-effective manner. CCCI will continue to be the
premiere provider of long-term care management to low-income elderly in
Connecticut.

Who We Are
CCCI is the premiere and largest care management service for elderly and
individuals with disabilities in Connecticut. We are dedicated to
assisting clients to remain independent and living at home for as long
as possible.
For more than 25 years, CCCI has helped thousands of individuals stay at
home through care management.

The Connecticut Home Care Program for Elders (CHCPE)
CCCI serves as an Access Agency to the public home care program in 124
towns throughout Northwest, North Central and Eastern Connecticut.
Care Management Associates
CCCI serves those who do not qualify for the public program through its private division.

families meet the multiple and varied needs of elders and individuals
