Clinical Ladders Program
The Clinical Ladders Program has been developed to increase the overall level of experience and proficiency of clinical staff. Advancement is based on a formal application and review process and includes self-directed team input in the evaluation process.

The program includes four levels:

Self-directed teams allow individuals with varied expertise and ideas to make decisions regarding their work processes and practice in a collective voice. Teams foster innovation and improve processes, providing a work environment of respect and accountability.

The basic goal of teams is to insure quality client outcomes. Team members are accountable for the responsibilities that enable CCCI to achieve its mission, vision and values. Members work in collaboration with each other in a collegial spirit to promote effective team structure, workflow, processes and outcomes.

Regional Supervisor in the following offices: Wethersfield

Connecticut Community Care, Inc has a full-time position in our Wethersfield office for a Regional Supervisor.  Experience outstanding personal and professional rewards as you assist in maintaining office procedures/practices, the quality improvement program, and supervise/ coordinate care management/office staff activities.

The selected candidate will have a Bachelor’s degree in nursing or a Master’s degree in health, social work, gerontology or a related human services field.  The nurse or social services worker shall have a minimum of two years of experience in health care or human services.  In addition, must have demonstrated supervisory ability and at least one year of specific experience in conducting assessments, developing care plans and monitoring home and community based services.
