Clinical Ladders Program
The Clinical Ladders Program has been developed to increase the overall
level of experience and proficiency of clinical staff. Advancement is
based on a formal application and review process and includes
self-directed team input in the evaluation process.
The program includes four levels:
- Care Manager in Training
- Care Manager I
- Care Manager II
- Care Manager III
Self-Directed Teams
Self-directed teams allow individuals with varied expertise and ideas to
make decisions regarding their work processes and practice in a
collective voice. Teams foster innovation and improve processes,
providing a work environment of respect and accountability.
The basic goal of teams is to insure quality client outcomes. Team
members are accountable for the responsibilities that enable CCCI to
achieve its mission, vision and values. Members work in collaboration
with each other in a collegial spirit to promote effective team
structure, workflow, processes and outcomes.
Positions Available
Care Manager in the following offices: Watertown
Successful candidates must possess either:
- a BSN with 1 year clinical experience or RN diploma with 2 years clinical experience and a current CT RN license.
- or: a BA or BS in social work, gerontology or a related human services field and 2 years clinical experience. MSW preferred.
Individuals must possess thorough knowledge of principles and practices of the individual’s specific social service or nursing discipline. Knowledge and understanding of psychological, social, health, and economic factors influencing the attitudes and behavior of individuals and families, especially as they relate to the elderly, knowledge and skill in interviewing and assessment techniques, understanding of terminal illness and its effect on the individual and family. Knowledge of the Spanish language is a plus. Ability to work in a team relationship essential. Computer experience is desired
Marketing Outreach Coordinator in the following offices: Bristol
Full-time opening for a Marketing Outreach Coordinator in our Bristol Office.
The Marketing Outreach Coordinator acts as an inter-departmental/organizational liaison with regards to maintaining an understanding of the organizations marketing needs. Establishes and maintains relationships with new and existing contacts as well as referral resources.
Education:
The selected candidate must possess a BA/BS in business, marketing, healthcare, or equivalent work experience.
Experience/Requirements:
