The Connecticut Home Care Program for Elders (CHCPE)
  • CCCI is a state Access Agency to the CHCPE.
  • We manage care for more than 7,400 program clients at any one time who meet the age, financial and functional requirements of the program.
  • Our mission is to help our clients live at home safely, independently and in the most cost-effective manner

How It Works


Professionally trained nursing and social services care managers provide:

Assessment


An in-depth evaluation of the client's current situation including their health, level of functioning, cognitive and emotional status, their living environment, financial situation and available support system.

Care Plan Development


An individualized cost-effective plan that addresses the current and long-term care needs of the client and a written report of findings that meet their personal or cultural preferences.

Coordination


Assistance with accessing the services necessary to meet the identified needs of the client. We also ensure the services have been implemented and are running smoothly. This includes coordinating and arranging for services such as home health agencies, homemakers, chore and companion services, adult day centers and more.

Monitoring


We monitor progress to ensure that the plan continues to meet the client's changing needs. Professional care managers are available 24/7 to help in emergencies and discuss concerns. Plans are modified as necessary.
