The Home Team: Planning and Assistance
This Home Team approach pairs the professional geriatric care manager,
who designs and closely monitors the client's care plan, with the
personal assistant, who helps clients with various activities of daily
living. It offers improved quality of care, consistency of care from
one organization and one-stop shopping for families.
CMA Care Managers provide planning services and:
