The Home Team: Planning and Assistance


This Home Team approach pairs the professional geriatric care manager, who designs and closely monitors the client's care plan, with the personal assistant, who helps clients with various activities of daily living. It offers improved quality of care, consistency of care from one organization and one-stop shopping for families.

CMA Care Managers provide planning services and:


  • Offer telephone or office consultation
  • Assess client's situations
  • Provide written reports of findings
  • Investigate resources and offer options to families
  • Provide access to and coordinate support services
  • Provide care management
  • Facilitate informed decision making
  • Handle things for the long-distance caregiver

CMA Personal Assistants can:


  • Schedule appointments
  • Arrange for transportation
  • Assist with bathing and dressing
  • Light housekeeping
