How We Can Help
Connecticut Community Care is Connecticut's premier and largest care
management service for the elderly. Our goal is to help clients live at home
safely and as independently as possible. In fact, as a private,
nonprofit, CCCI has helped thousands of elders and individuals with
disabilities live safely and independently at home for more than 25
years. Professionally trained nursing and social service care managers
help elders and individuals with disabilities find services and
assistance from paid service providers. Care managers look at our clients' full
range of needs to find the right resources and to coordinate the care
program. They also arrange services for family caregivers. When you ask us
to evaluate a client or loved one, you can be sure they receive a
comprehensive and individual evaluation. We typically receive referrals
directly from family members, friends, physicians, attorneys, bank trust
officers, financial planners, social workers, probate judges and others.
Programs and How They Work
CCCI services can be accessed via one of two programs. The state
program, The Connecticut Home Care Program for Elders (CHCPE), requires
qualification. The Care Management Associates (CMA) program is a
fee-for-service program open to all. See below for more information.
