For Providers
As an Access Agency contracting with the Connecticut Department of Social Services to provide and arrange for the provision of services to elderly clients on the Connecticut Home Care Program For Elders in Northwest, North Central and Eastern Connecticut, CCCI contracts with direct service providers to deliver services to clients. We have procedures and mechanisms in place to screen, contract with and monitor services and analyze the quality of care provided.
CCCI contracts with the following types of providers:
- Home Health Care Agencies Licensed by the CT Dept. of Public Health
- Adult Day Centers Certified by the CT Adult Day Care Association
- Companion & Homemaker Providers Registered with the Dept. of Consumer Protection
- Home Delivered Meals Providers
- Emergency Response Service Providers
- Transportation Providers
- Mental Health Counseling Providers
CHCPE pays providers for the following services:
- Skilled Nursing
- Home Health Aide
- PT, OT, ST
- Mental Health Counseling
- Adult Day Care
- Homemaker
- Companion
- Chore
- Home Delivered Meals
- Emergency Response System
- Minor Home Modification
Potential providers provide CCCI information such as:
- Licensures/certifications/registrations
- Quality Assurance Plan
- Insurances Carried
- Bonding
- Hiring practices
- Proof of financial stability
- EEOC compliance
- Compliance with local, state and Federal Labor Laws
- Description of services
- Rates including usual & customary
- Towns served
- Ability to serve non-English speaking clients
- References
- Ongoing education (training)
Providers are required to maintain the following types of records:
- Individual client records
- Records of service delivery
- Financials including Audited Statements
- Documentation regarding employees
CCCI is currently seeking providers in the following areas:
- Adult Day Center---All Regions
- Home Health Care agencies---All Regions
- Homemaker Companion Agencies---Eastern and Northwest Regions
- Mental Health Counselors---All Regions
Rates paid to providers are set by the Department of Social Services.
